Ritalin shows promise in treating addiction

June 27, 2013 ? A single dose of a commonly-prescribed attention deficit hyperactivity disorder (ADHD) drug helps improve brain function in cocaine addiction, according to an imaging study conducted by researchers from the Icahn School of Medicine at Mount Sinai. Methylphenidate (brand name Ritalin?) modified connectivity in certain brain circuits that underlie self-control and craving among cocaine-addicted individuals.

The research is published in the current issue of JAMA Psychiatry, a JAMA network publication.

Previous research has shown that oral methylphenidate improved brain function in cocaine users performing specific cognitive tasks such as ignoring emotionally distracting words and resolving a cognitive conflict. Similar to cocaine, methylphenidate increases dopamine (and norepinephrine) activity in the brain, but, administered orally, takes longer to reach peak effect, consistent with a lower potential for abuse. By extending dopamine's action, the drug enhances signaling to improve several cognitive functions, including information processing and attention.

"Orally administered methylphenidate increases dopamine in the brain, similar to cocaine, but without the strong addictive properties," said Rita Goldstein, PhD, Professor of Psychiatry at Mount Sinai, who led the research while at Brookhaven National Laboratory (BNL) in New York. "We wanted to determine whether such substitutive properties, which are helpful in other replacement therapies such as using nicotine gum instead of smoking cigarettes or methadone instead of heroin, would play a role in enhancing brain connectivity between regions of potential importance for intervention in cocaine addiction."

Anna Konova, a doctoral candidate at Stony Brook University, who was first author on this manuscript, added, "Using fMRI, we found that methylphenidate did indeed have a beneficial impact on the connectivity between several brain centers associated with addiction."

Dr. Goldstein and her team recruited 18 cocaine addicted individuals, who were randomized to receive an oral dose of methylphenidate or placebo. The researchers used functional magnetic resonance imaging (fMRI) to measure the strength of connectivity in particular brain circuits known to play a role in addiction before and during peak drug effects. They also assessed each subject's severity of addiction to see if this had any bearing on the results.

Methylphenidate decreased connectivity between areas of the brain that have been strongly implicated in the formation of habits, including compulsive drug seeking and craving. The scans also showed that methylphenidate strengthened connectivity between several brain regions involved in regulating emotions and exerting control over behaviors -- connections previously reported to be disrupted in cocaine addiction.

"The benefits of methylphenidate were present after only one dose, indicating that this drug has significant potential as a treatment add-on for addiction to cocaine and possibly other stimulants," said Dr. Goldstein. "This is a preliminary study, but the findings are exciting and warrant further exploration, particularly in conjunction with cognitive behavioral therapy or cognitive remediation."

Additional co-authors included Nora D. Volkow, MD, the Director of the National Institute on Drug Abuse; Dardo Tomasi, the MR director at BNL; and Scott J. Moeller, PhD a postdoctoral fellow in psychiatry at Mount Sinai.

(AP) ? South Africans were torn on Wednesday between the desire not to lose a critically ill Nelson Mandela, who defined the aspirations of so many of his compatriots, and resignation that the beloved former prisoner and president is approaching the end of his life.

The sense of anticipation and foreboding about 94-year-old Mandela's fate has grown since late Sunday, when the South African government declared that the condition of the statesman, who was rushed to a hospital in Pretoria on June 8, had deteriorated.

A tide of emotional tributes has built on social media and in hand-written messages and flowers laid outside the hospital and Mandela's home. On Wednesday, about 20 children from a day care center posted a hand-made card outside the hospital and recited a poem.

"Hold on, old man," was one of the lines in the Zulu poem, according to the South African Press Association.

In recent days, international leaders, celebrities, athletes and others have praised Mandela, not just as the man who steered South Africa through its tense transition from white racist rule to democracy two decades ago, but as a universal symbol of sacrifice and reconciliation.

In South Africa's Eastern Cape province, where Mandela grew up, a traditional leader said the time was near for Mandela, who is also known by his clan name, Madiba.

"I am of the view that if Madiba is no longer enjoying life, and is on life support systems, and is not appreciating what is happening around him, I think the good Lord should take the decision to put him out of his suffering," said the tribal chief, Phathekile Holomisa.

"I did speak to two of his family members, and of course, they are in a lot of pain, and wish that a miracle might happen, that he recovers again, and he becomes his old self again," he said. "But at the same time they are aware there is a limit what miracles you can have."

For many South Africans, Mandela's decline is a far more personal matter, echoing the protracted and emotionally draining process of losing one of their own elderly relatives.

One nugget of wisdom about the arc of life and death came from Matthew Rusznyah, a 9-year-old boy who stopped outside Mandela's home in the Johannesburg neighborhood of Houghton to show his appreciation.

"We came because we care about Mandela being sick, and we wish we could put a stop to it, like snap our fingers," he said. "But we can't. It's how life works."

His mother, Lee Rusznyah, said Mandela, who spent 27 years in prison under apartheid before becoming South Africa's first black president in all-race elections in 1994, had made the world a better place.

On Tuesday, a South African archbishop who visited Mandela offered a prayer in which he wished for a "peaceful, perfect, end" for the anti-apartheid leader, who was taken to the Pretoria hospital to be treated for what the government said was a recurring lung infection.

In the prayer, Thabo Makgoba, the Anglican archbishop of Cape Town, asked for courage to be granted to Mandela's wife, Graca Machel, and others who love him "at this hard time of watching and waiting," and he appealed for divine help for the medical team treating Mandela.

"May your blessing rest upon Madiba now and always," Makgoba said in the prayer. "Grant him, we pray, a quiet night and a peaceful, perfect, end."

He wished that Mandela would be granted relief from pain and suffering, and also said: "Uphold all of us with your steadfast love so that we may be filled with gratitude for all the good that he has done for us and for our nation, and may honor his legacy through our lives."

Visitors to the hospital on Wednesday included Mandela's former wife, Winnie Madikizela-Mandela. The couple divorced in 1996.

Mandela, whose 95th birthday is on July 18, served a single five-year term as president and afterward focused on charitable causes, but he withdrew from public life years ago and became increasingly frail in recent years. He last made a public appearance in 2010 at the World Cup soccer tournament, which was hosted by South Africa. At that time, he did not speak to the crowd and was bundled against the cold in a stadium full of fans.

On April 29, state television broadcast footage of a visit by President Jacob Zuma and other leaders of the ruling party, the African National Congress, to Mandela's home. Zuma said at the time that Mandela was in good shape, but the footage ? the first public images of Mandela in nearly a year ? showed him silent and unresponsive, even when Zuma tried to hold his hand.

Before the weekend announcement that Mandela was in critical condition, the South African government, former leader Thabo Mbeki and at least two members of Mandela's family had said his health was improving. The divergence between such upbeat reports and what appears to be a more dire reality has contributed to a sense of uncertainty, even as the government says the privacy of Mandela and his family must be respected.

"Let's accept instead of crying," said Lucas Aedwaba, a security officer in Pretoria who described Mandela as a hero. "Let's celebrate that the old man lived and left his legacy."

Dan Lehman, an American academic, chose a jogging route on Wednesday morning that passed by the hospital where Mandela is being treated.

"I was just going out for my morning run down here and come to pay my respects to the greatest man in the world," Lehman said. Then he began to cry.

For staging, some rooms and issues can present more challenges than others. An overpopulated living room, an untidy bedroom, an outdated kitchen, and a bathroom with more rings around it than Saturn can make for a staging nightmare.

Karen Eubank of Eubank Staging has seen all these issues and more. So we asked her??What are some of the biggest problem areas when staging a home for today?s market??

Here?s her thoughtful answer:

The ?problem room? varies from house to house.

The single biggest ?issues? for any room are clutter, cleanliness and light. Oh and it is absolutely inexcusable to have anything on your property that needs to be repaired. Do all of the repairs prior to listing.

People in general have a lot of items, I have a lot of items. I?d need the entire Real Estate Staging Association board to help me stage my home if I ever moved! The good news is you are moving, so go ahead and start packing and store the items your stager suggests.? You can bet that will include any sort of collection you have.

Seldom do we keep our homes ?white glove? clean. I?m notorious for simply putting 25 watt bulbs in all the light fixtures prior to a party so I don?t have to clean. Try it, it works! Instant mood lighting and no need to clean!
However when it?s the big, bad, broad light of day you have to have a super clean house. It needs to not only BE clean but smell clean.

Light is a huge factor. It really helps sell a home. So open blinds, curtains, shutters and make sure the light bulbs are working in all of your fixtures. One big tip to make sure ceiling can light bulbs are the same depth within each fixture. If you have one low and one high the light distribution will be uneven.

Buyers will make a decision within 15 seconds about purchasing a home. That means your bang for the buck needs to happen in the areas you see from the front door. Walk and count. Everything you see in those first 15 seconds needs to say
?This is the house!?.

Kitchens are generally an issue if they are dated. Wallpaper, odd color cabinets, linoleum flooring and of course bad lighting need to be addressed. Buyers want a great kitchen and great bathrooms. Just look at remodeling statistics and you will see that both of those items bring more ROI than other things you can do to a home. I?m not saying remodel before you sell, just be aware of the facts. People generally spend a lot of time in their kitchens, they are the heart of the home and buyers don?t want to have to move in and do heart surgery!

For instance, no one wants to see a washer and dryer in a kitchen. Remember that great Lake Highlands house Kerry Slaughter listed last year? I suggested they build doors over their washer/ dryer area. The seller did and the house flew off the market.That was before the market was hot. Kerry and I would both say that issue would have been a big barrier to selling the home.

A super quick fix in a typical ranch is to paint the cabinets (Tricorn Black by Sherwin Williams is a great choice ? yes BLACK), update? cabinet hardware and light fixtures and the entire kitchen will have a new personality! Oh and that lovely decorative edging around ranch house kitchen windows comes right off! Pick a nice wall color like Agreeable Gray or Kilim Beige and make it a kitchen every buyer will love.

A big deal killer is a home without a proper master bathroom. That will absolutely lower your price. If you have a great master buyers will be more forgiving of a second bath that is not updated. White is always right when it comes to fixtures.? Re-glazing is cost effective for tubs and sinks.? Now if you live in an? vintage home known for purple or pink toilets with multicolor wall tile? leave it alone. That is exactly what buyers in that area are looking for!

One of the biggest issues it the ?mult-task?room. So often the guest room is also the workout room and the craft room. This is disastrous when you sell. Each room has to have clearly defined purpose. If you have a three bedroom home, one bedroom can certainly be a home office but it cannot also be a gym and an art? studio.? It has to be ONE thing. Do not confuse your potential buyers. They are looking at several homes a day and if you want yours to be THE home, they need to remember it without any possibility? of confusion. If a buyer is trying to recall a home and says ?Oh yeah, the cute house with the umm office, no was it a gym or no I think it was a craft room?? That?s a house that will not be first on the list.?

Endgame at hand for immigration bill in Senate

WASHINGTON (AP) ? The Senate is preparing to push landmark immigration legislation across a few final hurdles and possibly pick up one or two more Republican "yes" votes along the way.

The legislation cleared an early procedural hurdle Monday with room to spare, and more test votes awaited Wednesday. The White House-backed bill would pour billions of dollars into border security and offer a path to citizenship to some 11 million immigrants now in the United States illegally.

After the addition of provisions greatly strengthening border security and doubling the size of the border patrol, the legislation looks likely to command support from at least 14 Republicans on final passage Thursday or Friday. That's more than enough to ensure the 60-vote margin needed for passage, as all 52 Democrats and the two independents who usually vote with them look likely to stick together.

Some supporters would like to bring even more Republicans on board and are working to negotiate votes on amendments to win over anyone who is wavering.

Sen. Rob. Portman, R-Ohio, is pushing an amendment to strengthen an electronic employment verification program made mandatory in the bill.

"I can't vote for (the bill) without it," Portman said.

Sen. Saxby Chambliss, R-Ga., wants changes to a new agriculture workers program that he says makes it too easy for farm workers to get permanent U.S. residency.

The measures sought by Portman and Chambliss are being opposed by some immigrant advocacy groups, and some Senate Democrats believe the bill has enough Republican support as is without pursuing more changes. But Sen. Chuck Schumer, D-N.Y., a lead sponsor of the bill, continues to push for further amendments, arguing that a large bipartisan vote in the Senate is necessary to pressure the GOP-controlled House to act.

In the House on Wednesday, the Judiciary Committee was to vote on its own employment verification legislation. It's the third in a series of single-issue immigration bills the committee has voted on as it takes a piecemeal approach to overhauling the nation's immigration system, in contrast with the Senate's comprehensive bill.

None of the measures considered by the House panel includes a path to citizenship or even legalization for the millions here illegally, something opposed by many House conservatives. That leaves the final fate of the immigration overhaul effort unclear once the spotlight moves from the Senate to the House after this week's action.

I Lost Weight: Anthony Williams Fell In Love With Running And Lost 105 Pounds

How I Gained It: I was always a heavier kid. There was a clear difference in my weight between my third and fourth grade class photos. As a child, I was much more content with staying inside and playing than going outside to play with friends. The most physical activity I ever did as a child was to ride my bicycle around the neighborhood. While the food options my parents provided were not healthy by today's standards, they were also not horrible. Yes, there was junk food around, but I was provided with fruits and vegetables as well. I honestly think it was more about how the items were prepared and how there was little limitation on my portion sizes. I do not feel that my parents are personally to blame as much as the environment in which we all grew up is. It is a vicious cycle that we need to focus on in this country.

Breaking Point: I remember it so vividly. I was on a trip home to visit my family, and I had to rush my father to the emergency room, as he was having some issues with his leg. He had already lost one leg due to complications with diabetes, and now his second leg was losing blood flow. In complete honesty, the sight of his leg on that evening literally made me sick to my stomach. I sat there thinking, "I don't know how these medical professionals can look at this and not be phased." It was on that night after the doctor informed my father that there was nothing else they could do for him and it was now necessary for them to remove his second leg. my life changed. It was at that exact moment that I realized that if I continued down my current path of eating habits and no exercise, I faced being in the exact same position as my father in 10 to 15 years, if not sooner.

How I Lost It: Having previously "dieted" numerous other times, I took some time to consider reasons I had failed in the past. I believe I failed so many times in the past because I attempted to "bite off more than I could chew." I opted to focus on my eating habits first. I made the commitment in June 2010 to begin documenting every little bite I ate. I am a very visual person, and the fact that any "bad choices" I made would be forever documented was a huge factor for me. After controlling my eating habits for a few months, I joined a gym, something I had never done in my previous attempts to lose weight. I made the sole focus of my visits to learn to run. I would hop on that treadmill and run for one minute, followed by two minutes of walking, and repeat that for 30 minutes. During each of those workouts I felt I was on the edge of death. I gradually worked my way to running for greater amounts of times -- two minutes, one whole song -- until I could run a whole mile without stopping. I committed to attending the gym every Sunday, Monday, Wednesday and Thursday. I would make sure that I packed my bag prior to leaving for work. I knew if I allowed myself to go home after work I would never get back out. In November of that same year, I signed up for a local Thanksgiving day race, which was 4.5 miles, but had a "shortcut" route of 2.3 miles. I did the shortcut, but still had to walk some of it. I didn't let that stop me though. I continued pushing myself. I completed my first 5K in May 2011 and ran the entire course without stopping.

Since that 5K, I have now completed six half-marathons, and in October 2012, I completed a full marathon. I run an average of 100 miles a month, and the foods I used to despise are now part of my regular intake. I have been able to keep my weight off, and I consider myself "addicted" to running. Actually, I seek out all types of exercise now, including biking, yoga and high intensity interval training. I feel unsatisfied if I do not complete some sort of exercise every day. I have even taken my love of running and turned it into a volunteer activity. I volunteer with an organization called Back On My Feet. It is a non-profit organization that promotes the self-sufficiency of homeless people by engaging them in running as a means to build confidence, strength and self-esteem. As member of the core team, I run with members of the program every Monday, Wednesday and Friday at 5:45 a.m., regardless of the weather conditions (well, for the most part!).

I know I am a different person now, because there is a sense of confidence I have in myself that I never had before. I am no longer ashamed of myself. I know I am a healthy and fit person who can accomplish anything I put my mind to.

From 2010 to 2011, enrollment of part-time graduate students in science and engineering (S&E) fields grew at a higher rate than that of full-time S&E graduate students for the first time since 2005.

The new finding comes from a report released today by the National Science Foundation that reveals that enrollment of part-time S&E graduate students increased 1.6 percent versus 0.5 percent for full-time S&E graduate students from 2010 to 2011.

During the past decade, enrollment of full-time graduate students in S&E grew almost 25 percent, from approximately 325,000 students in 2002 to approximately 411,200 students in 2011. Enrollment of part-time students increased nearly 15 percent, from approximately 129,300 students in 2002 to approximately 149,700 students in 2011.

Romo update adds telepresence

Romo first rolled into our lives via Kickstarter. Since then we've seen it return leaner and meaner, with more improvements just announced. Romotive tells us that an app update coming today brings full telepresence functionality, allowing users to log into the device from anywhere via any iOS device or PC running a Chrome browser. Setting up the telepresence should be no harder than setting up a regular call, and once you're set, you'll get two-way video and audio -- plus control of the robot (including its expressions!). This not only gives Romo new scope for (almost literally) becoming one of the family, it adds a whole host of new use cases. Want a few ideas to get you started? Militants kill 9 foreign tourists, 1 Pakistani

ISLAMABAD (AP) ? At least a dozen Islamic militants wearing police uniforms shot to death nine foreign tourists and one Pakistani before dawn Sunday as they were visiting one of the world's highest mountains in a remote area of northern Pakistan that has been largely peaceful, officials said.

The foreigners who were killed included five Ukrainians, three Chinese and one Russian, said Pakistani Interior Minister Chaudhry Nisar Ali Khan. One Chinese tourist was wounded in the attack and was rescued, he said.

The local branch of the Taliban took responsibility for the killings, saying it was to avenge the death of a leader killed in a recent U.S. drone strike.

The shooting was one of the worst attacks on foreigners in Pakistan in recent years and is likely to damage the country's already struggling tourism industry. Pakistan's mountainous north ? considered until now relatively safe ? is one of the main attractions in a country beset with insurgency and other political instability.

The attack took place at the base camp of Nanga Parbat, the ninth highest mountain in the world at 8,126 meters (26,660 feet). Nanga Parbat is notoriously difficult to climb and is known as the "killer mountain" because of numerous mountaineering deaths in the past. It's unclear if the tourists were planning to climb the mountain or were just visiting the base camp, which is located in the Gilgit-Baltistan region of Pakistan.

The gunmen were wearing uniforms used by the Gilgit Scouts, a paramilitary police force that patrols the area, said the interior minister. The attackers abducted two local guides to find their way to the remote base camp. One of the guides was killed in the shooting, and the other has been detained and is being questioned, said Khan.

"The purpose of this attack was to give a message to the world that Pakistan is unsafe for travel," said the interior minister in a speech in the National Assembly, which passed a resolution condemning the incident. "The government will take all measures to ensure the safety of foreign tourists."

Pakistani Taliban spokesman Ahsanullah Ahsan claimed responsibility for the attack, saying their Jundul Hafsa group carried out the shooting as retaliation for the death of the Taliban's deputy leader, Waliur Rehman, in a U.S. drone attack on May 29.

"By killing foreigners, we wanted to give a message to the world to play their role in bringing an end to the drone attacks," Ahsan told The Associated Press by telephone from an undisclosed location.

At least a dozen gunmen were involved in the attack, local police officer Jahangir Khan said.

The attackers beat up the Pakistanis who were accompanying the tourists, took their money and tied them up, said a senior local government official. They checked the identities of the Pakistanis and shot to death one of them, possibly because he was a minority Shiite Muslim, said the official, speaking on condition of anonymity because he was not authorized to talk to reporters.

Although Gilgit-Baltistan is a relatively peaceful area, it has experienced attacks by radical Sunni Muslims on Shiites in recent years.

The attackers took the money and passports from the foreigners and then gunned them down, said the official. It's unclear how the Chinese tourist who was rescued managed to avoid being killed. The base camp has basic wooden huts, but most tourists choose to sleep in their own tents.

Local police chief Barkat Ali said they first learned of the attack when one of the local guides called the police station around 1 a.m. on Sunday. The military airlifted the bodies to Pakistan's capital, Islamabad, Sunday afternoon.

"We hope Pakistani authorities will do their best to find the culprits of this crime," the Ukrainian ambassador to Pakistan, Volodymyr Lakomov, told reporters outside the hospital where the bodies were taken.

The Pakistani government condemned the "brutal act of terrorism" in a statement sent to reporters.

"Those who have committed this heinous crime seem to be attempting to disrupt the growing relations of Pakistan with China and other friendly countries," said a statement issued by the Foreign Ministry.

Pakistan has very close ties with neighboring China and is sensitive to any issue that could harm the relationship. Pakistani officials have reached out to representatives from China and Ukraine to convey their sympathies, the Foreign Ministry said.

Many foreign tourists stay away from Pakistan because of the perceived danger of visiting a country that is home to a large number of Islamic militant groups, such as the Taliban and al-Qaida, which mostly reside in the northwest near the Afghan border. A relatively small number of intrepid foreigners visit Gilgit-Baltistan during the summer to marvel at the peaks of the Himalayan and Karakoram ranges, including K2, the second highest mountain in the world.

Syed Mehdi Shah, the chief minister of Gilgit-Baltistan, condemned the attack and expressed fear that it would seriously damage the region's tourism industry.

"A lot of tourists come to this area in the summer, and our local people work to earn money from these people," said Shah. "This will not only affect our area, but will adversely affect all of Pakistan."

The area has been cordoned off by police and paramilitary soldiers, and a military helicopter was searching the area, said Shah.

"God willing we will find the perpetrators of this tragic incident," said Shah.

The government suspended the chief secretary and top police chief in Gilgit-Baltistan following the attack and ordered an inquiry into the incident, said Khan, the interior minister.
